Collection Instructions:

1. Specimen should be drawn by CentraCare Laboratory Services staff. Exceptions: NICU, Surgery, Homecare, CT..

2. Label specimen with patient’s name (first and last), medical record number and/or DOB, date and time of draw, and initials of phlebotomist.

3. Plasma gel tube is not acceptable.

4. Specimen cannot be frozen.
